For enrollment contact us and we will schedule a time for you and your child to come in and meet with us learn more about our program and get a chance to see our home and what we have to offer. During the interview I will be happy to go over any questions you may have. A packet of necessary forms will be given and must be completed and returned before your child can begin care at Happy Kids Home Daycare.

Listed below you will find the list of forms that you need to return to Happy Kids Home Daycare before or on your child’s first day.

The following records will be kept on file for each child enrolled in the program.

DCFS Required-Child registration letter

DCFS Required-pick –up agreement

DCFS Required-Consent to provider to get medical care, consent to administer patent medication, consent to administer, prescription medication, consent for excursions and trips and consent to use park facilities.

DCFS Required-medical report up to date and no more than 6 mos old with Immunization record.

DCFS Required-Licensing standards for home daycares (signature page)

DCFS Required- Copy of child’s birth certificate

Signed parent provider agreement

Written permission to transport child

Permission to photograph
